Estes Park Health serves the entire Estes Park Valley and Rocky Mountain National Park. It integrates acute and emergency care with community health, which includes the full range of diagnostic and therapeutic health care through medical and surgical services; a 23-bed critical access acute care facility, a 24-hour emergency department with ambulance service and air transport; primary care and specialty clinics; home health and hospice; and Urgent Care Services.

The Clinical Coordinator is responsible for ensuring that patient care is coordinated and managed appropriately; responsible for ensuring that care and services are delivered appropriately and for the supervision of clinical personnel

SHIFT: Full time, days.  Hours may vary.

  • B.S.N. graduate of an N.L.N. accredited school of nursing.
  • Current licensure in state, American Heart Association CPR certification and valid driver’s license.
  • Minimum of two years computer literacy.


  • Maintains a professional demeanor as evidenced by appearance, dress, no body odor, attitude in contacts with other staff, physicians, patients and their support systems.
  • Utilizes good time management techniques to maintain productivity level as determined by agency.
  • The ability to be reliable and punctual for work and for each visit.
  • Use the nursing process to meet the needs of patients of all ages.
  • Assists in Clinical Management of all nursing care given to Home Care/Hospice patients.
  • Assess care given by HHA’s and intervene as necessary to maintain quality care. Assist with Supervision, orientation, in-services, skills labs, assess charting administer skills checklist for HHA.  Evaluate HHA skill level.
  • Coordinate communication with physicians in order to facilitate patient care coordination.
  • Communicate relevant information to physicians.
  • Assists Director with special projects, program implementation as assigned.
  • Acts as a Clinical resource for RN’s. Facilitates care of difficult/complicated patients.
  • Provides RN/CNA staff scheduling including on-call schedule.
  • Maintains current CPR, skills checklist.
  • Takes intake referrals from all other agencies, refers people to other resources when Estes Park Home Care cannot provide appropriate levels of care. Assess patients to insure eligibility for services, performs financial review and insurance verification.
  • Assists in performing Infection Control Audits as related to home care setting. Performs QI for the department as Chair of CQI Committee.  Delegates CQI activities to appropriate staff members.
  • Assists in planning in-services education to meet the needs of nursing personnel, therapists and CNA’s.
  • Assists with review of all charting as necessary to insure quality care.
  • Facilitates coordination of patient information to all concerned in an efficient manner to include case conferences, patient review, team meetings.
  • Ability to carry own case load as dictated by staffing levels.
  • Demonstrates commitment to professional growth and competency.
  • Promotes positive interpersonal relationships and demonstrates effective communication skills.
  • Maintains confidentiality of all patients and Home Care/Hospice information.
  • Practices according to professional and ethical standards. Demonstrates an understanding of and functions as a professional nurse in accordance with Colorado State Nurse Practice Act.
  • Provides care to patients in Home Care, Hospice, Lifeline and Home OB. The nurse will demonstrate competency in providing care for and case manage to all types of patients including high tech, hospice, complicated social issues and those requiring a variety of disciplines.  This list is NOT all inclusive.
  • Complete legible, written visit report.
  • Communicate clearly report to RN’s and other staff.
  • Attends staff meetings and/or reads minutes, patient review.
  • Ability to work in an environment of strict confidentiality and maintain such an environment, both at work and in off hours
  • Participate in committees as assigned.
  • Perform in accordance with all local, state, and federal laws and regulatory agency standards
  • Perform in accordance with EPH vision, mission and goals
  • All other duties as assigned

Hiring range depending on experience is $33.66 to $44.88.
