Estes Park Health serves the entire Estes Park Valley and Rocky Mountain National Park. It integrates acute and emergency care with community health, which includes the full range of diagnostic and therapeutic health care through medical and surgical services; a 23-bed critical access acute care facility, a 24-hour emergency department with ambulance service and air transport; primary care and specialty clinics; home health and hospice; and Urgent Care Services.

Estes Park Health Home Health Services can provide a single service or a combination of services, medical and non-medical in a patient’s home. Our qualified staff plans, coordinates and provides care tailored to individual needs.
Estes Park Health Hospice Care offers a multidisciplinary team approach to working with individuals and families to care for anyone with a life-limiting illness. The hospice team includes the patient, family, medical director, nurses, social worker, aides, volunteers and chaplain. We hold contracts with pharmacies and medical equipment suppliers to adequately meet patient needs.
The Home Health / Hospice RN completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness (es).
SHIFT:
PRN(as needed)
- Graduate of an accredited school of nursing. One (1) to two (2) years of recent acute care experience in an institutional setting. Bachelor’s degree, with one (1) year of home health care experience preferred.
- Management experience not required. Responsible for supervising home health aides.
- Current licensure in state, American Heart Association (AHA) Provider BLS certification and valid driver’s license.
- Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. Considerable walking and standing. Able to lift, position and transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling and crouching. Visual acuity and hearing to perform required nursing skills.
Hiring range depending on experience $34.14-$46.43, plus hourly PRN differential $4.50.
