Estes Park Health serves the entire Estes Park Valley and Rocky Mountain National Park. It integrates acute and emergency care with community health, which includes the full range of diagnostic and therapeutic health care through medical and surgical services; a 23-bed critical access acute care facility, a 24-hour emergency department with ambulance service and air transport; primary care and specialty clinics; home health and hospice; and Urgent Care Services.

Estes Park Health Home Health Services can provide a single service or a combination of services, medical and non-medical in a patient’s home. Our qualified staff plans, coordinates and provides care tailored to individual needs.  

Estes Park Health Hospice Care offers a multidisciplinary team approach to working with individuals and families to care for anyone with a life-limiting illness. The hospice team includes the patient, family, medical director, nurses, social worker, aides, volunteers and chaplain. We hold contracts with pharmacies and medical equipment suppliers to adequately meet patient needs.

The Personal Care Provider is a member of the home care team who works under the supervision of a supervisor or agency director and performs various personal care services as necessary to meet the patient’s needs. The personal care provider will be assigned in a manner that promotes quality, continuity and safety of a patient’s care.

SHIFT:

Full Time, 8 hour day shifts, times may vary, days may vary. *The shift and hours listed above may be subject to temporary or permanent change at any time according to the needs required to provide excellent and full services to our customers during employment.

POSITION REQUIREMENTS:

  • High School or equivalent.
  • Meets the training requirements in accordance with state and federal laws.
  • At least 18 years of age.
  • Licensed driver with automobile that is insured in accordance with Organization requirements and is in good working order.
  • American Heart Association BLS certification.
  • Ability to read and follow written instructions and document care given. Self-directing with the ability to work with little direct supervision. Has empathy for the needs of the ill, injured, frail and the impaired. Demonstrates tact, patience and good personal hygiene.

DUTIES:

  • Providing personal care including: Bathing, Skin care, Hair care, Nail care, Shaving, Positioning, Bladder and bowel care, Medication reminding, Oral hygiene, Changing bed linen, Assisting patients with dressing and undressing, Skin care to prevent breakdown, Keeping patient’s living area clean and orderly, as appropriate.
  • Planning and preparing nutritious meals.
  • Assisting in feeding the patient, if necessary.
  • Assistance in completing activities such as shopping.
  • Companionship including, but not limited to social interaction, conversation, emotional reassurance, encouragement of reading, writing and activities that stimulate the mind.
  • Assistance with activities of daily living, personal care and any other assignments as included in the service plan.
  • Completion of appropriate service notes regarding service provision each visit. Documentation shall contain services provided, date and time in and out, and a confirmation that care was provided.
  • Assisting in ambulation and exercise according to the plan of care.
  • Assisting patient in the self-administration of medication.
  • Doing patient’s laundry, as appropriate.
  • Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.).
  • Maintains a professional demeanor as evidenced by appearance, attitude with other staff including positive and effective interpersonal relationship and communication skills.
  • Adhering to the Organization’s documentation and care procedures and standards of personal and professional conduct.
  • Participate in committees as assigned.
  • Perform in accordance with all local, state, and federal laws and regulatory agency standard.
  • Perform in accordance with EPH vision, mission and goals.
  • All other duties as assigned

Hiring range depending on experience is $13.24 – 17.65 

$500 Sign-on Retention Bonus. $250 on first paycheck and $250 after 12 months.
