The Estes Park Health Transitional/Swing Bed Care provides nursing care for patients who require medically supervised nursing care on a continuous basis, but no longer require their previous level of acute care. Since acute care hospital stays are frequently short-term, patients can benefit from additional care days in the Transitional/Swing Bed Care Unit.
If you are admitted to Estes Park Health as a hospital patient, you can convert to the Transitional/Swing Bed Care Unit and remain in your same room and bed. If you have received care at another hospital and are in need of skilled-nursing services, you can be transferred to Estes Park Health's Transitional/Swing Bed Care Unit.
Please contact our Case Manager at 970-235-3007 for assistance.
