Estes Park Health Home Health Services can provide a single service or a combination of services, medical and non-medical in a patient’s home. Our qualified staff plans, coordinates and provides care tailored to individual needs.

Le Centre de soins palliatifs Estes Park Health Hospice Care offre une approche d'équipe multidisciplinaire pour travailler avec les personnes et les familles afin de prendre soin de toute personne atteinte d'une maladie limitant l'espérance de vie. L'équipe du centre de soins palliatifs comprend le patient, la famille, le directeur médical, les infirmières, le travailleur social, les aides, les bénévoles et l'aumônier. Nous détenons des contrats avec des pharmacies et des fournisseurs d'équipement médical afin de répondre adéquatement aux besoins des patients.

 The Occupational Therapist evaluates patient’s functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, work capacity, etc.).  Evaluates home environment for hazards or barriers to more independent living.  Identifies equipment needs.  Participates in the development of the total plan of care. For patients who plan to return to work, the occupational therapist may perform work capacity evaluation and may refer to specialized vocational training in accordance with Organization policy.

SHIFT :

PRN, days as needed, 8 0r 10 hrs days.

LES EXIGENCES DU POSTE :

  • Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association. Registered with the National Registration Examination of the American Occupational Therapy Association.
  • Two years of clinical experience as an occupational therapist.
  • Possess and maintains current American Heart Association CPR Certification.
  • Demonstrates good verbal and written communication and organization skills.
  • Understands hospice philosophy; issues of death/dying.
